VNUS Medical Technologies ("VNUS") makes the Physician Locator Service (“Service”) available for your use on this Website ("Site") subject to the terms and conditions set forth in this document and any changes to this document made by VNUS from time to time (collectively, "Terms and Conditions"). By using the Service, you agree to these Terms and Conditions. If you do not agree to these Terms and Conditions, you may not use the Service.

VNUS agrees that any such surveys, questionnaires, etc. will be reasonable in number and length.
Follow-Up by Physician
By checking the appropriate box(es) when using the Service, you agree to allow the corresponding physician(s) or medical practice group(s) to contact you regarding possible treatment for venous disorders.

Term; Termination
These Terms and Conditions are effective until terminated by VNUS.
